Health Promotion & Disease Prevention

Community Health brings communities together to combat chronic disease
through multiple, interlocking strategies between health, social
service, education, and faith partners. We offer intensive,
results-oriented technical assistance and grants management to grantees
across the state. We are pleased to provide the management team for
chronic disease prevention and reduction to two of North Carolina's
most important foundations, the Kate B. Reynolds Charitable Trust and
the North Carolina Health and Wellness Trust Fund. And we advise and
support low-income communities in both North and South Carolina, in
partnership with the University of South Carolina School of Medicine,
to improve nutrition and reduce obesity. And we provide on-line
low-literacy modules in English and Spanish, with companion support
groups, throughout the Southeast to family caregivers caring for family
