Durham Community Health Network

Durham Community Health Network (DCHN) is a community-based managed care program for Durham Carolina Access II enrollees funded by the State of North Carolina. DCHN is governed by a network of agencies and providers who serve the Medicaid population, including: primary care clinics, local hospitals, and the Durham Departments of Social Services and Health. The program is administered by the Duke Division of Community Health. DCHN helps primary care practices improve the quality of care that Carolina Access patients receive by removing barriers to healthcare access, helping patients better understand their healthcare conditions and responsibilities, sharing best practices. The case management team includes clinical personnel, social workers and community/lay health workers.

Pharmacy Home Initiative
Pharmacy costs represent one of the greatest areas of increased expense in the Medicaid budget. The Pharmacy Home Initiative provides local networks with additional resources to educate providers about quality and cost effective prescribing strategies. The Clinical Pharmacist working with DCHN also conducts medication therapy management reviews to assure that patients' medications are both cost-effective and best suited to their health conditions.

Chronic Care Initiative
The Chronic Care Initiative focuses on the aged, blind and disabled category of Medicaid recipients. This population of Medicaid recipients frequently has complex healthcare needs. The population represents nearly 70% of the state's annual Medicaid expenditures. Funding from this initiative has allowed DCHN to provide this population with enhanced screening and assessment for services that may allow them to live more safely and healthfully in their community. These services include in-home occupational therapy assessment without having a hospitalization to promote safety and independence, nutrition counseling, mental health screening and referral, medication therapy management review and education, and nurse case management.
