Clinical & Care Management Programs
Duke
Community Health's clinical services are delivered by
multi-disciplinary teams in community settings. They are configured and
delivered together with our community partners. We count our partners'
contributed staff as part of our staff, working in local sites with our
patients. Our clinical care has financing strategies built on these
partnerships which reduce expenses and add revenue to make our programs
sustainable.
We provide primary care without regard to ability
to pay in three types of settings: neighborhood-based clinics,
school-based clinics, and in-home medical care and case management.
Community Health offers in-home care management, which includes health
education, patient advocacy, and patient support, to both Medicaid and
uninsured patients. Duke Community Health's programs include:
